Exparel® (Bupivacaine Liposome Injectable Suspension) – 13.3 mg/mL, 20 mL Vial

Description:
Exparel® is a long-acting, non-opioid local anesthetic indicated for single-dose infiltration to provide extended postoperative analgesia. Formulated as a liposome-encapsulated bupivacaine, Exparel® delivers sustained pain relief for up to 72 hours after surgical procedures, helping to reduce or eliminate the need for opioid analgesics.

Strength & Formulation:

  • Active Ingredient: Bupivacaine (liposome injectable suspension)

  • Form: White to off-white milky suspension for injection

Clinical Applications:
Exparel® is commonly used for:

  • Soft tissue surgical procedures, including oral and maxillofacial surgeries (e.g., wisdom tooth extraction, implant surgery)

  • Orthopedic, general, and plastic surgeries requiring prolonged analgesia

  • Field blocks, including intercostal nerve block, for postsurgical pain control

Administration Guidelines:
Exparel® is administered via local infiltration into the surgical site and may be combined with bupivacaine HCl (within dosing limits) for immediate onset. It must not be mixed directly with non-bupivacaine local anesthetics, as this may affect the liposomal integrity and drug release.

Warnings and Precautions:

  • For use by healthcare professionals experienced in regional anesthesia techniques

  • Avoid intravascular administration; aspirate frequently to prevent toxicity

  • Monitor for signs of systemic toxicity, especially with higher doses

  • Not recommended for obstetric paracervical block or intravenous regional anesthesia
